Please bring the required documents listed below:
*Guardian Identification
* Birth certificate
*Immunization Record
*All 7th grade students MUST have the Tdap vaccine to enroll or Certificate of Exemption https://oklahoma.gov/content/d...
*Proof of residency *gas, water, or electric bill- Must be current (last 30 days)
*Legal documents (if applicable) divorce/custody, guardianship, foster care, legal name change.

Athletic physicals are scheduled for Monday, May 13th @6:00pm in the High School Gym. Doctors fee is $25 cash.
If your child intends on participating in athletics grades 7-12, they must have a physical on file. Physical forms will be provided to your child from their coach. Parent signatures are required. If you choose, you may use your own doctor.
